Analysis Assessment

  • An Analysis Assessment is required prior to completing the first Membership Session. 

  • A card on file is required to reserve your appointment slot.

  • There is a 24-hour cancellation policy or the assessment rate will be charged via the card on file.Please arrive on time for your Analysis Assessment. The Analysis Assessment can last up to 90 minutes from the start time of your appointment. 

P4Gā„¢ Personal Training Package Purchase

  • No initiation fees.

  • No REFUNDS.

  • All memberships run auto-monthly. Memberships will be processed on the date of joining. Your auto-renewal date will be the date of your first session and processed on this date going forward. Sessions expire one month from the date of your membership start/renewal date and do not roll over.

30-Day Notice Cancellation Policy

  • All memberships process auto-monthly and require a 30-day notice via email to be terminated. For example, send an email by November 31st so that your subscription does not go through on January 1st. Please email info@performforgolf.com to request termination.

Vacations/Time off/Pauses Policy

  • Membership Pauses are on a MONTH TO MONTH basis. Please email info@performforgolf.com to fulfill this request. You must request each month you would like to pause; otherwise your Membership will automatically reactivate. It is required to email  info@performforgolf.com at least 14 days in advance of your auto-renewal date regarding skipping an auto-monthly payment due to travel, etc. If you plan to extend your Membership Pause for 2 or more consecutive months, you will need to reach back out again at least 14 days in advance of your auto-renewal date to info@performforgolf.com to let the admin team know. They will help you with your pauses respectively. In the process of extending a Membership Pause 2 or more consecutive months, you may be subject to purchase a non-automonthly session package membership option. If a member pauses for 3+ months, they will be subject to loss of membership. Furthermore, said member will have to resume membership under the new client pricing. Members are welcome at any time to downgrade their membership if they would still like to have access to our services. For memberships with personal training access, if you travel for more than 1 month, your regular weekly time slot may be replaced.

24-Hour Late Cancel/No-Show Policy

  • All reserved sessions have a 24-hour window for canceling session(s).

  • For late cancellations/no-shows/reschedules for personal training and/or bodywork sessions, the session will be counted as used.
